A person or entity who mails or delivers, using commercial courier, overnight or other delivery services, contact lenses to patients in Kansas pursuant to a contact lens prescription which such person or entity did not determine. This does not apply to persons licensed to practice optometry or licensed to practice medicine and surgery in Kansas. See K.S.A. 65-4967(a).

For an initial registration, an application can be submitted online or by paper. Additional forms and documents will be required, and all fees must be paid.

The Board accept checks, money orders, Debit, Visa, MasterCard, Discover and American Express. To pay with debit or credit card, please use the Credit/Debit Card Payment Authorization Form.

The application process takes on average four to eight weeks; however, each application is different. Please keep in mind that information required for licensure or registration comes from the applicant and various outside organizations.

All contact lens distributor registrations expire 1 year from the date issued; the registration must be renewed annually.

The renewal cycle is 30 days prior to expiration. A renewal application will be emailed at the beginning of the renewal cycle.

Licensing Fees

On the day following expiration, a registration status is Cancelled – Failure to Renew. Practicing after a registration is cancelled may implicate disciplinary action.

A registration application must be completed, and the associated reinstatement fee(s) must be paid. Depending on the circumstances, reinstatement may require additional testing, training, or education as the board deems necessary.
