The Board accept checks, money orders, Debit, Visa, MasterCard, Discover and American Express. To pay with debit or credit card, please use the Credit/Debit Card Payment Authorization Form.

The application process takes on average four to eight weeks; however, each application is different. Please keep in mind that information required for licensure comes from the applicant and various outside organizations.

An applicant who meets all the requirements for licensure, except examination, is eligible for a temporary permit. Only one temporary permit may be issued to a person and expires one year after the date of issuance. To receive a temporary permit, an applicant must apply for one, and pay an additional $25 nonrefundable fee.

Wallet cards are emailed after the license has been issued and can be accessed anytime in the online portal.

All Occupational Therapist licenses expire on March 31st; the license must be renewed annually.

The renewal cycle for Occupational Therapists begins February 15th and ends April 30th each calendar year. To renew, login to the online portal during the renewal cycle.

The late renewal cycle is from April 1st – April 30th. There is an additional late fee.

On May 1st, the license status will be Cancelled – Failure to Renew. Practicing after a license is cancelled may subject an Occupational Therapist to disciplinary action.

A reinstatement application can be submitted and associated reinstatement fee(s) must be paid. Depending on the circumstances, reinstatement may require additional testing, training, or education as the board deems necessary.

Licensing Forms

Yes, K.A.R. 100-54-13 requires Occupational Therapists licensed in Kansas to maintain professional liability insurance. Individual coverage of not less than $100,000 per claim, and not less than $300,000 for all claims made during the period of coverage; or coverage through the individual’s employer under an additional insured policy of not less than $1,000,000 per claim, and not less than $3,000,000 for all claims made during the period of coverage.

An address may be changed in the online portal or by completing the Address Change Form.

A Change of Name Form should be submitted along with required documentation. To obtain a certificate with a new name, a Duplicate Certificate Request Form should be completed.
